Any application that is received without all the proper documentation will not be considered.

Additional Documentation to Be Sent With Application:

(You do not have to add us to your insurance policies until approved.  However, you must show evidence you have them)

• General Liability Insurance With At Least $1,000,000 Coverage
• Workman's Compensation Insurance With At Least $500,00 Coverage or Workman's Comp Exempt Form
• Automobile Insurance Policy With At Least $500,000 Coverage
• W-9 Form
• Business License

Upon acceptance of your application additional information will be required to prove your legal working status.  Also, all vendors and field employees are subject to a criminal background check that is paid for upfront by the vendor upon acceptance.
